Submit a Claim

The only way to receive a cash payment from this Settlement is by submitting a valid and timely Claim Form. The deadline to submit a Claim Form is January 22, 2027.

If you are submitting a claim by U.S. mail, the completed and signed Claim Form, along with any supporting documentation, must be mailed so it is postmarked no later than January 22, 2027 to:
